Private Tutoring Agreement
Guarantees and Conditions:
Please read the following paragraph carefully. You must check the I Agree box or your registration will not be considered valid.
I agree to the suggested hours of tutoring as per my conversation with the Lentz & Lentz representative. Should the tutor arrive for a prescheduled appointment, and the student fail to attend, Lentz & Lentz will be paid for the 90-minute session. Once the credit card has been processed, there will be a $25 processing fee in the event the student or parent decides not to be tutored. Once the designated date and time is set up with the tutor, Lentz & Lentz requires a minimum of twenty-four hours advance notice in order to cancel the appointment. If twenty-four hours advance notice is not received by email or telephone directly to Lentz & Lentz, the client will be charged for the full 90-minute session. If the student chooses to cut short a 90-minute session once the tutor has arrived, the client will still be charged for the entire 90-minute session. No credits will be given in cases of this nature. Once tutoring sessions have been completed, there are no refunds. If tutoring is done at home, we require a parent or legal guardian to be present.
PSAT/SAT/ACT PRIVATE TUTORING RATES FOR ONE STUDENT:
$145 per hour with a 9-hour minimum requirement (for any student who has not previously completed our standard 8-session SAT preparatory group course).
Sessions are conducted in 90-minute increments.
Based on availability and preference, the tutor will either come to the client's home, set up a virtual tutoring session, or schedule an appointment to meet at a mutually convenient place.
With minimum requirement, tutoring totals $1305 for 9 hours.
I acknowledge that I am the parent or legal guardian registering the student and I am at least 21 years of age.
